The skilled rehabilitation program at The Edgewood Centre provides treatment, care, and rehabilitation therapy to residents following a stay in the hospital, usually for orthopedic surgery, cardiac care, or a serious illness.
The goal of the program is to provide the highest quality of rehabilitative and nursing care to residents while facilitating their transition, as quickly and safely as possible, from hospital to home. When a resident is ready to leave Edgewood, we provide the support and services needed for a smooth transition home.
Comprehensive Care
Cost effective and convenient, the skilled rehabilitation program at Edgewood offers a wide range of short-term rehab and medical services for orthopedic, cardiac, neurological, pulmonary, and medically complex conditions. What’s more, skilled rehab and nursing care are provided with the special warmth, attentiveness, and focus on the full range of personal needs that have become Edgewood’s hallmark during more than three decades as a leader in the field of nursing care.
Orthopedic Rehab
The orthopedic rehabilitation program at The Edgewood Centre focuses on restoring musculo-skeletal function in individuals following joint replacements, fractures, back injuries, and amputations. An interdisciplinary team of nurses, physical and occupational therapists, and discharge planners work together to assess resident needs and develop individualized plans of care. The mission of the team is always to expedite recovery, maximize functional independence, and facilitate a safe discharge back home.
Cardiac Care
Multidisciplinary expertise in Edgewood’s cardiac care program integrates rehabilitation, nursing, nutritional management, medical care, and discharge planning. The team successfully treats residents with cardiovascular diagnoses, congestive heart failure, and post-cardiac surgery needs. Staff work to reduce pain and discomfort while encouraging the healing process and promoting optimal recovery. This program is highly regarded in the cardiac community, providing comprehensive medical and nursing care in a warm and supportive environment.
Neurological Rehab
The professionals that make up Edgewood’s neurological rehab team identify physical, cognitive, and psychological impairments and assess their impact on the resident’s ability to achieve his or her desired treatment goals. The program is designed for people who have spinal cord injuries, Parkinson’s disease, stroke-related deficits, traumatic brain injury, cerebral palsy, multiple sclerosis, and other neurological impairments. The neurological rehab team is committed to helping residents regain functional mobility, maximize motor control, improve communication skills, and optimize independence with self-care and daily living skills.
Pulmonary Rehabilitation
The pulmonary rehab program at Edgewood offers an interdisciplinary team of nursing and rehab professionals who treat people with COPD, pneumonia, post thoracic surgery, and other pulmonary issues. The program focuses on decreasing pain and discomfort, maximizing cardio-pulmonary function, optimizing independence and mobility, preventing recurrence of disease, and assisting with safe return home.
Medically Complex Program
Edgewood’s medically complex program is administered by an interdisciplinary team of medical, skilled nursing, and rehab professionals. The team assesses each resident’s needs and implements a plan of care that promotes optimal recovery. Residents with diabetes, cancer, post-surgical complications, gastro-intestinal problems, chronic renal failure, wound care, and vascular disorders are successfully treated in this program.
Family Involvement
At The Edgewood Centre we realize that a key component of residents’ success in attaining their goal to return home is the involvement of their family. To help promote residents’ recovery, we invite families to actively participate in resident care and discharge planning meetings. Working directly with the resident and family, a social worker is available to assist with insurance issues, help with discharge planning and support individual treatment goals. In addition, our team of therapists will do a home safety assessment and provide educational materials so that residents can more effectively manage a safe transition home after a skilled rehab stay.
Medicare Coverage
Admissions are temporary for those who require short term rehab after surgery or an extended hospital stay. The structure and support provided with 24 hour skilled nursing services enable individuals to maximize their level of independence and afford them the opportunity to return home safely. Skilled nursing care can be covered by the Medicare program if the resident meets the medical criteria. Typically Medicare covers only short-term stays limited to a maximum of 100 days.
