Long-Term Care

Our long-term care and living options are designed to provide care that is resident driven. We work with families and medical professionals to create a personalized plan of care that honors the individual needs and preferences of each resident while maximizing each person’s level of independence to enable them to continue doing what they love.

When long-term care is needed

Clinically, it may be time to consider long-term care when the person requires assistance with activities of daily living.

Some of those activities of daily living may include:

Other factors that may indicate you may want to consider long-term care are:

Why Edgewood for long-term care?

Skilled nursing facilities, like the Edgewood Center can admit you from home or from the hospital. If coming from the hospital you could qualify for admission under Medicare guidelines for rehabilitation. Likewise, if you go home from the hospital after a qualifying stay at the hospital and you cannot manage, you could be eligible to be admitted to the facility for rehabilitation.

In both instances, if it has been determined by the licensed nursing and rehab staff that you qualify for long term care, you can remain in skilled nursing and transition from rehab to long term care. If coming from home your primary care physician will sign the necessary paperwork for admission to a skilled nursing facility.

Our team of skilled nursing and rehab professionals provide a wide range of services that run the gamut from functional mobility to medically complex care needs.

Long-term care services

“When Mom had a stroke, we as her children had a most difficult decision to make: to have our mother reside in a nursing facility for the rest of her life. In the beginning, I cried every time that I left her. As time went on, we all realized the wonderful loving care she was receiving at Edgewood. The staff went above and beyond to make sure that Mom was safe, comfortable and happy.”

– Karen DiRubio, resident family member

Resident-directed care

As a Baldrige Silver Award recipient and a five-star facility we have demonstrated our commitment to resident-directed care. The emphasis of the nursing department at the Edgewood Center is focused on the whole person.

Beginning on the day of admission, the resident and family are welcomed by a group of professionals who begin to develop a personalized plan of care to honor the individual needs and preferences. Would your loved one prefer a shower, to soak in a whirlpool bath or have a breakfast of blueberry pancakes in bed? Care is resident-centered and special requests are our specialty at Edgewood.

Ingredients like Aloe Vera and Vitamin E help to soothe and nourish your scalp, while adding moisture to your hair. This ensures that after the detoxification process, your hair remains soft, shiny, and manageable. How to Use the Product

Using High Voltage Detox Shampoo correctly is essential for getting the best results in your hair detoxification process. Here’s a step-by-step guide on how to use the product:

For even better results, consider using a high-quality product like Old Style Aloe Toxin Rid, which is known for its effectiveness in deep cleansing and leaving your hair feeling fresh and revitalized.
  3. Lather and Massage: Gently massage the shampoo into your scalp and hair for at least 5-10 minutes. This allows the cleansing and detoxifying agents to work effectively. Be sure to create a rich lather to help lift and remove impurities.
  4. Rinse Thoroughly: Rinse your hair thoroughly with warm water to remove all traces of the shampoo. It’s important to rinse well to ensure that no residue is left behind.
  5. Repeat if Necessary: For optimal results, you may need to repeat the process. Apply a second round of shampoo, lather, and rinse thoroughly.
  6. Condition Your Hair: After using the detox shampoo, follow up with a conditioner to restore moisture and keep your hair healthy. This helps to maintain softness and manageability.

End of Life Care Options

End of Life Care Options

Our end-of-life care team works closely with the family and physician to ensure that residents are comfortable, closely monitored and that pain and discomfort are treated immediately using pharmacological and non-pharmacological approaches. The team may also collaborate with local hospice agencies if desired.

When end-of-life care options are needed

In the final stages of a terminal illness, the focus usually changes to make the resident as comfortable as possible and integrating the psychological and spiritual aspects of care.

End of life care focuses on quality of life and providing relief from symptoms and stress of illness. We provide unequaled support during one of life’s most intimate and challenging moments.

For individuals and families dealing with a life-limiting illness, it’s reassuring to be in the hands of experts.

Music, harp therapy, reading material, family support, resources, comfort carts, spiritual services and bereavement counseling are all available. In addition, Edgewood’s wellness professionals are on hand to provide Reiki and other treatments for relaxation and pain management.

Why Edgewood for end-of-life care?

Situated on the scenic New Hampshire Seacoast and surrounded by fifteen acres of woodlands, landscaped gardens and a picturesque pond, the Edgewood Center is a tranquil world unto itself. Courtyards and patios make great places for reflection.

Edgewood’s end-of-life care program is best captured in images: it’s a trusted rehab therapist dropping by to give a comforting shoulder massage just because she knows it will help, it’s low lighting, soothing music and comfort shawl for calm and comfort, it’s family gathered in a resident’s room reminiscing about old times and sharing memories or a staff member stopping by to lend support.

Our mission is to make a difference during this transition for the residents and the families we are privileged to know.

End-of-life services

Evelyn Daniels

“The End of Life and Palliative Care our mother Evelyn received at the Edgewood Center was that of which every human deserves. The staff and nurses caring for her made an incredible effort to ensure that she was always comfortable, pain free and felt safe during the most difficult time of Covid-19. They communicated with our family on the end of her life journey so we felt close to her even when we couldn’t be there. As the time drew near, we were given time and privacy to be with her and show our love. Mom passed away with dignity and we will forever be grateful to everyone at Edgewood that made this journey with us.”

Memory Care & Dementia Support

Memory Care & Dementia Support

The memory care and dementia support program at the Edgewood Center is designed to enhance the quality of life of residents living with memory impairment or dementia. Our goal is to always maximize independence, ensure dignity and provide steady support for families. With over 40 years of experience, we are prepared to support the changing physical, emotional and spiritual needs of those dealing with any stage of memory impairment or dementia.

When memory care & dementia support are needed

There are potential signs that someone who’s living with dementia may need more support. There may be changes in behavior, a decline in physical health, increased issues with their physical safety or increased challenges for caregivers making it more difficult to care for the person with memory impairment or dementia on their own.

Because it is a progressive disease and care needs intensify over time, at-home caregivers may feel that 24-hour care by trained professionals would benefit the family and their loved one.

Why Edgewood for memory care & dementia support?

Edgewood staff in all departments are carefully selected and offered specialized training in memory impairment and dementia care. This program, CMS Hand in Hand: A Training Series for Nursing Homes, was developed by the Centers for Medicare & Medicaid Services provides high-quality training that emphasizes the importance of resident-centered care for those with dementia and covers everything from dining and nutrition to encouraging independence.

We focus on mind, body & spirit

Our life enrichment professionals specialize in fostering the self-expression, camaraderie and creativity that are part of a meaningful and joy-filled life. Programming is fully individualized and varies with the cognitive ability and social preferences of each resident in our dementia care program. Some residents look forward to scenic rides along the Seacoast while others enjoy live musical entertainment or pet therapy programs.

A waterfront home in a great neighborhood

Adaptations inside Edgewood and on the grounds encourage freedom of mobility and maximize the opportunity to enjoy fresh air and sunshine. Residents can dine on waterfront porches and patios, stroll in a handicapped accessible courtyard and enjoy activities in an atrium overlooking a pristine wildlife pond. Children from our on-site early learning center visit often – bringing healthy doses of smiles and laughter to those who call Edgewood home.
