We work with families and medical professionals to create a personalized plan of care that honors the individual needs and preferences of each resident while maximizing each person’s level of independence to enable them to continue doing what they love.
When long-term care is needed
Clinically, it may be time to consider long-term care when the person requires assistance with activities of daily living.
Some of those activities of daily living may include:
- Medication management
- Bathing
- Dressing
- Toileting
- Transferring from one surface to another (in and out of bed would be an example)
Other factors that may indicate you may want to consider long-term care are:
- Home management concerns: meal preparation, housekeeping, laundry
- Physical challenges: balance or memory issues
- Safety concerns: leaving doors unlocked, leaving the stove unattended
- Social isolation: loneliness, fear of being alone
- Caregiver burnout: lack of availability of caregivers
- Failure to thrive: depression, limited social interaction
Why Edgewood for long-term care?
Skilled nursing facilities, like the Edgewood Center can admit you from home or from the hospital. If coming from the hospital you could qualify for admission under Medicare guidelines for rehabilitation. Likewise, if you go home from the hospital after a qualifying stay at the hospital and you cannot manage, you could be eligible to be admitted to the facility for rehabilitation.
In both instances, if it has been determined by the licensed nursing and rehab staff that you qualify for long term care, you can remain in skilled nursing and transition from rehab to long term care. If coming from home your primary care physician will sign the necessary paperwork for admission to a skilled nursing facility.
