End of Life Care

As part of our skilled nursing care program, The Edgewood Centre in Portsmouth, NH offers end of life care for individuals and families dealing with a life limiting illness.

The Palliative Team

When it comes to palliative care, it’s reassuring to be in the hands of experts. Edgewood’s Medical Director, Dr. Sarah MacDuffie, is board certified in hospice and palliative medicine and is the Palliative Care Director at Portsmouth Regional Hospital. Working alongside Dr. MacDuffie is an interdisciplinary palliative care team made up of nursing, medical, rehab, activities, and social service professionals who provide residents and families with the support they need during difficult times.  Music, reading material, music, journals, family support, spiritual services, and bereavement counseling are all available.

In addition, Edgewood’s wellness professionals are on hand to provide reiki and massage for relaxation and pain management. The palliative team works closely with local hospice agencies to ensure that residents are comfortable, closely monitored, and that pain and discomfort are treated immediately using pharmacological and non-pharmacological approaches.

Award Winning New Hampshire Skilled Nursing Care

One of Edgewood’s caregivers, Candy Kuchman, was honored as New Hampshire Licensed Nursing Assistant of the year for her exemplary work in the field of long-term care.

Candy is just one example of the many fine nursing professionals who staff The Edgewood Centre. These men and women combine technical skill and nursing expertise with the warmth and compassion that is essential in end of life nursing care.

Beacon Hospice presented Edgewood with their End of Life Care Award for 2011. Beacon chose Edgewood based on a staff of highly compassionate caregivers who are professional and dedicated to honoring individual resident choices.

All of Edgewood’s staff people play an integral part in the care and support of residents accessing hospice services, allowing families and residents to direct their care at end of life.

An End of Life Care Snapshot

Edgewood’s hospice program is best captured in images: It’s a trusted rehab therapist dropping by to give a shoulder massage just because she knows it will help, it’s children from Edgewood’s on-site early learning center parading down the hall dressed as bunnies for Easter, it’s family gathered in a resident’s room reminiscing about old times and capturing them on film or in a journal, and it’s an Edgewood chef preparing a resident’s favorite meal and serving it to the entire family.
